Charitable Support

You have been directed to this page after receiving a text message confirming your appointment date to meet with a social worker.

Charitable support requests are handled by
Sidra Medicine’s Social Work Department

For the appointment with the social worker, you will need to bring all the documents necessary to process the application with the charity.  This includes:

• Download Required Documents for Charity
• Download Charity Application form

Please print and complete the charity application form and gather all documents listed prior to your appointment.

If you are unable to print the application form you can collect one from any of the information desks located on the plaza level in the outpatient clinic or hospital buildings.
