Overall Learning Objectives

  1. Understand the purpose and evidence base for fiberoptic endoscopic evaluation of swallowing (FEES)
  2. Understand appropriate patient selection, considering safety, risks and benefits
  3. Understand elements of a comprehensive FEES examination and tailoring to the individual’s needs

Time Session Title/Topic Session-Specific Learning Objectives
8-8:15 Welcome and Introductions
8:15-9:15  FEES History, Literature review and Differences with VFSS

Understand the purpose and evidence base for FEES and VFSS

Understand the indications and contraindications for FEES

9:15-9:45 Laryngeal anatomy, structure and function Understand the anatomy of the larynx in relation to swallowing
9:45-10:00
10:00-10:15
10:15-11:00 Normal FEES and how structural anomalies alter this  (e.g VCP, cleft, laryngomalacia, TEF, tracheostomy in-situ)

Describe:

  1. Elements of what a normal FEES looks
  2. Altered FEES in relation to structural anomalies
1100-1145 Standardization of FEES reporting Understand components of detailed reporting using examples such as the Airway Protection, Penetration Aspiration scale (PAS), Yale residue scale, Reflux Finding Score etc.
11:45-12:00
12:00-1:00
1-2 How to prepare a pediatric patient for a successful study and FEES, including breastfeeding and neonatal FEES

Understand the elements of a comprehensive FEES examination

Understand safety considerations – high-risk and vulnerable patient populations, specifically for neonates, tracheostomized patients, patient with a vocal cord palsy etc.

2-2:45 Scoping skills (discussion with videos) Understand the technique for performing FEES as an endoscopist, including experience with local anesthesia and nasal decongestants.
2:45-3
3-4 Implementing a FEES clinic/Dysphagia Program (interactive)

Explain previous experience in setting up a referral procedure.

Identify components necessary to implement a FEES service / FEES clinic / dysphagia program (ENT SLP MDT).

Time Session Title/Topic Session-Specific Learning Objectives

8-10

Hands-on session
(with multiple hands-on stations)

Describe anatomical landmarks viewed endoscopically that contribute to infant/pediatric dysphagia

Describe altered anatomy/physiology and possible impact on swallowing function observed during infant and pediatric FEES

10-10:30
10:30-12:30

Case discussions

  • Specific anatomic scenarios: laryngeal clefts, vocal cord paralysis, laryngomalacia, trachea-esophageal fistula
Understand how interpretation of FEES is tailored to the background anatomical anomaly

Interpretation of FEES findings within the context of medical and dysphagia history, previous swallowing assessments, severity and prognostication.

12:30-1:30
2:30-3:30 Surgical treatment options tailored to findings (laryngeal cleft repair, supraglottoplasty, TVF injection, TEF closure, etc) Describe the surgical options available for laryngeal clefts, laryngomalacia, vocal cord palsy and trachea-esophageal fistula etc.
