الدوحة، قطر، 09 ديسمبر 2019 : صغر صيوان الأذن ھو تشوه خلقي يؤثر على نحو واحد من بين كل 4,000 مولود عالميًا 1. والأطفال المولودون بھذا المرض تكون لھم آذان غير مكتملة التكوين أو لا تكون لھم آذان تمامًا.
ليس ثمة سبب معروف لصغر صيوان الأذن ويمكن أن يؤثر على واحدة من الأذنين أو كليھما (صغر صيوان الأذن ثنائي الجانب). وغالبًا ما يعاني الأطفال المصابون بصغر صيوان الأذن من درجات مختلفة من فقد السمع نتيجة قصور نمو منطقتي الأذن الخارجية والوسطى أو عدم وجودھما.
Dr. Mitchell Stotland, Division Chief of Plastic and Craniofacial Surgery, leads Sidra Medicine’s world-class microtia reconstruction program. The program also covers children with the associated condition of hemifacial microsomia, including with interventions to augment and/or lengthen the undergrown tissues and bones of the face. The multidisciplinary medical/surgical team is supported by Sidra Medicine’s Allied Health services for all forms of craniofacial disorders.
Dr. Stotland said: “In the past, microtia and hemifacial microsomia patients in Qatar were referred to hospitals in Europe or North America. Or experts from abroad were occasionally brought in for a few days to specifically treat the microtia. With the opening of our main hospital facility in January 2018, the Plastic and Craniofacial Surgery Division at Sidra Medicine began to offer many specialized pediatric services here in Qatar. The care we provide is done within a multidisciplinary context, which ensures the continuum of care for the patient. We have assembled a wonderful program with a world-class team. I also believe that what we provide now at Sidra Medicine for patients with microtia is on par with the finest care that anywhere in the world.”
وتنطوي عملية رأب صغر صيوان الأذن على استخدام قطع من الغضروف يتم الحصول عليھا من صدر المريض. ويتم بعد ذلك تھذيب الغضروف ونحته ثم تركيب الأذن بدقة متناھية لتوافق الأذن الأخرى للطفل، أو ربما أذني أحد الوالدين لأولئك الأطفال الذين يعانون من فقد الأذن على كلا جانبي الرأس.
ويشير د. ستوتلاند بالتحديد إلى إنشاء الأذنين لمرضى صغر صيوان الأذن قائلاً: "لقد أجرينا كذلك للعديد من الأطفال عمليات إطالة للفك وتكبير للنسيج الرخو. وتعدّ ھذه العملية، أكثر من أي جراحة أخرى على الإطلاق، مزيجًا بين العلم والفن. ويعتمد نجاح ھذه العمليات في أغلبه على التخطيط المتأني والتصوير الطبي عالي الجودة واستخدام القوالب الجراحية المخصصة والرعاية المتنبھة لما بعد الجراحة لضمان التعافي على أفضل وجه".
خضع عبد الرحمن، أحد مرضى صغر صيوان الأذن، لجراحة رأب للأذن مؤخرًا في سدرة للطب. وتعليقًا على الجراحة، قالت الأم ماريبل: "نحن مبھورون بتوفر ھذه الخدمة في سدرة للطب. وقد كان أداء د. ستوتلاند وفريقه رائعًا، حيث وضحوا لنا تفاصيل كل خطوة من خطوات الجراحات والرعاية التالية للجراحة في المنزل. يحب عبد الرحمن أذنه الجديدة وقد رأيته ينظر إليھا بانبھار في المرآة عدة مرات حيث لا يصدّق أنھا مصنوعة من غضاريف صدره! وأنا شخصيًا أعرف الأثر الإيجابي الذي ستتركه ھذه الجراحة على ابني، خاصة عندما ينمو ويكبر. فھي ستخلّصه من مشكلات الخجل والثقة بالنفس التي كنا قلقين جدًا من أنه سيواجھھا إذا لم تُجر له ھذه الجراحة في الوقت المناسب".
Most surgical interventions for patients with microtia are performed after the age of four to five. Total ear reconstruction does not usually occur prior to seven to eight years of age, when a child is big enough to provide adequate tissue for a surgeon to construct an ear. Microtia reconstruction typically requires two to three procedures. If a patient also has hemifacial microsomia, additional interventions are usually beneficial.
